CHEST reserves the right to edit letters for length and clarity. In reference to the very interesting report by Spira and colleagues, 1 which appeared recently in CHEST, I would like to add another illness to Dr. Spira's list: Chagas' disease. This is an infection, endemic in Central and South America, mostly in the central heartlands of Brazil, caused by a protozoan, Trypanosoma cruzi. In its chronic stage, it causes a well-known myocardiopa-thy, as well as often very prominent esophageal and colonic lesions, the " megas. " Its bronchopulmonary manifestations have been recently and extensively reviewed. 2 T cruzi infects wild animals, such as armadillos, racoons, and rodents, as well as humans. Transmission is by the blood sucking beetles of the Triatominae subfamily, which live in the cracks of mud-walled housing; by night, they bite their victims, usually in uncovered areas, mostly the face, originating their vulgar name of " barbei-ros " (barbers). In the United States, the disease may be seen in immigrants 3 or be acquired by congenital or blood transfusion transmission. Chagas' disease may course with an acute phase, seen mostly in children, with fever and inflammation at the inoculation site. Occasionally, there is severe homolateral palpebral inflammation (Romañ a's sign). The chronic stage is usually marked by the myocardial lesions, as well as the esophageal and colonic dilata-tions. These produce severe symptoms (dysphagia, constipation) and typical radiologic and endoscopic findings. The myocardio-pathy is characterized by arrhythmias and bundle-branch blocks, usually with severe and intractable heart failure at the late stages. The anatomic basis for all of these lesions is fundamentally similar. There is a gradual destruction of nerve cells in the digestive tube walls and of myocardial myofibrils by T cruzi Leishmania forms. The lungs may …

Effects of two new dihydropyridine derivatives, mepudipine and dibudipine, on isolated human internal mammary artery and rat left atrium
Among the present classes of calcium channel blockers, dihydropyridine derivatives are widely used in the therapy of hypertension, angina pectoris and the other cardiovascular diseases. Since the prototype of dihydropyridine derivatives, nifedipine, does not have the optimum pharmacokinetic and pharmacodynamic characteristics, several attempts have been made to synthesize other drugs in this ...

Technical aspects of composite arterial grafting with double skeleton-ized internal thoracic arteries. Bilateral skeleton-ised internal thoracic artery graftings in off-pump coronary artery bypass: early result of Y versus in situ grafts. results of coronary grafting using ultrasonically skeletonized internal thoracic arteries. متن کاملA unilateral approach to bilateral thoracoscopic internal mammary artery harvesting.
A technique of harvesting both internal mammary arteries thoracoscopically using three unilateral ports is described. Bilateral thoracoscopic internal mammary artery (IMA) harvesting was performed using a unilateral approach in 18 patients. The mean harvest time for harvesting was 52.3 +/- 17.5 min for the contralateral LIMA and 35.6 +/- 6.7 min for the ipsilateral RIMA. The mean free flow for ...
متن کاملEndoscopic harvesting of the left internal mammary artery.
Minimally invasive coronary artery bypass grafting via left anterior small thoracotomy is routinely performed on patients with single coronary artery disease, but recently has been expanded to a larger population as a part of a hybrid treatment in multivessel coronary artery disease. While the methods of internal mammary artery harvesting used in these operations can be different, the endoscopi...
متن کاملMusculoskeletal and neurological complications following coronary artery bypass graft surgery: A comparison between saphenous vein and internal mammary artery grafting.
The internal (thoracic) mammary artery (IMA) is currently the vessel of choice for coronary artery bypass grafting (CABG), due to its long term patency. The purpose of this study was to compare the incidence and nature of musculoskeletal and neurological complications following saphenous vein grafting (SVG) and internal mammary artery grafting (IMAG). Ninety-seven patients were screened by a ph...
متن کاملMammary artery harvesting using the Da Vinci Si robotic system
Internal mammary artery harvesting is an essential part of any coronary artery bypass operation. Totally endoscopic coronary artery bypass graft surgery has become reality in many centers as a safe and effective alternative to conventional surgery in selected patients. Internal mammary artery harvesting is the initial part of the procedure and should be performed equally safely if one wants to ...
